Salary: $15.00 - $16.00/hr DOE

Job Description:

Assist Health Services staff with care coordination, scheduling, medical interpretation, and transportation. This position will typically spend at least 50% of working time in the office and up to 50% directly with clients and community partners, particularly health services providers. Assist other programs as required with interpretation, including Job Development, Case Management, and Refugee Foster Care.

 

This position focuses on one of these three areas and includes some or all of the following.

  1. Tracking, coordinating, and implementing children's immunizations so that all clients under the age of 18 stay up-to-date with their vaccine schedule and may remain in school and be eligible for their green cards.
  2. Working closely with the Health Screening Coordinator to ensure successful completion of PPD skin tests, follow-up blood and lab work, and chest x-rays. This individual will work closely with the Salt Lake County Health Department to assist with tracking, coordination, and successful completion of LTBI treatment for eligible CCS clients
  3. The coordination of dental services for eligible refugee clients including reaching out to community partners to create an oral health curriculum and coordinate dental services for Medicaid clients.

Responsibilities include:

  1. Coordinate ongoing medical care for 600 refugee clients per fiscal year. Coordinate with multiple calendars and stakeholders to avoid scheduling conflicts and no-shows at appointments.
  2. Provide interpretation, transportation, and related assistance for refugee clients with Health Screening appointments, follow-up primary and specialized care appointments, and throughout our health education curriculum
  3. Complete all necessary documentation for appointments and services provided including billing forms, case noting, vehicle reporting, etc. accurately and in a timely manner.
  4. Attend regularly scheduled case management and staff meetings.
  5. Perform other duties as assigned.

Work Environment inside/outside, office, warehouse, noise, temperature, etc.

  1. This position is in a professional office environment using standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines.

Physical Demands including bending, sitting, lifting and driving

  1. This is largely a sedentary role; however, some filing is required. This would require the ability to lift files, open filing cabinets, and bend or stand as necessary.
  2. This role includes driving locally to apartment complexes and meetings.

  3. Able to be sensitive to the needs of clients, client's families, visitors, co-workers, volunteers, and other persons with whom the employee may interact.

  5. Adheres to deadlines.
  6. Proficiency in English oral and written communication.
  7. Must maintain confidentiality regarding clients.
  8. Proficiency in computer skills to maintain case files, and e-mail in Outlook.
  9. Personal qualities of openness and respect for co-workers and clients,

Education, training and or experience:  

  1. Bachelor's degree from an accredited institution or equivalent experience.
  2. Familiar with and supportive of the mission of Catholic Community Services of Utah and the vision of the Catholic Church articulated by the diocesan bishop.
  3. Experience with multi-cultural and refugee community.

Background Screen/BCI requirement

  1. Must pass a national BCI/FBI background check.

Licenses, certifications and credentials

  1. Current Utah driver's license, proof of auto insurance with minimum policy liability limits of $50,000 per person and $100,000 per occurrence, 21 years of age or older, a clean driving record, and ability to meet CCS vehicle safety requirements.

Position status and expected schedule  

  1. This is a full-time, 40 hours per week position.
